Are you a motivated and compassionate Occupational Therapist looking to make a meaningful impact—while enjoying the autonomy and flexibility of caring for patients in their homes? At Care New England, we’re seeking dedicated OTs to join our Home Health team and deliver personalized, goal-driven care that empowers patients to live as independently and safely as possible.


What You’ll Do

As a Home Health Occupational Therapist, you’ll manage a caseload of patients from admission through discharge, designing and implementing treatment plans that address individual needs and goals. You’ll collaborate with an interdisciplinary team and support patients through education, adaptive strategies, and therapeutic interventions—all from the comfort of their own home.


Your Day-to-Day Might Include

  • Conducting comprehensive assessments and functional evaluations
  • Developing personalized treatment plans focused on independence and safety
  • Implementing therapy sessions to improve mobility, strength, and daily living skills
  • Educating patients and caregivers on exercises, equipment use, and home safety
  • Adjusting care plans based on patient progress and clinical findings
  • Collaborating with nurses, physical therapists, speech therapists, and case managers
  • Documenting care in accordance with agency policies and regulatory standards
  • Participating in team meetings and quality improvement initiatives

What You’ll Need

  • Education and credentials required for RI Occupational Therapy licensure (required)
  • Bachelor’s Degree in Occupational Therapy or related discipline (strongly preferred)
  • Valid Rhode Island OT license and CPR certification (required)
  • At least one year of progressive clinical experience (home health or acute care preferred)
  • Valid driver’s license and automobile liability insurance
  • Excellent clinical, documentation, and communication skills
