Band 6 Emergency Department Discharge Team Specialist Physiotherapist

Do you want to extend your acute clinical skills via a static emergency department discharge team (EDDT) post in a forward thinking department? Then you could be the person for us. We are looking for an enthusiastic and dynamic individual to join our physiotherapy team at Queen Elizabeth Hospital, Greenwich.

• Provide a clinical service to a varied patient caseload as part of the multidisciplinary team (MDT) at Lewisham and Greenwich NHS Trust.
• Work closely with occupational therapists and develop generic therapy skills to support patient flow.
• Gain experience in Emergency Department, Ambulatory care, Frailty Ward, and Medical Admissions Unit.
• Be involved in supervision and teaching of Band 5 physiotherapists, Therapy Assistants and students as required.
• Contribute to service development within the EDDT, including cross-site development as needed.

The post offers opportunities to build on existing clinical knowledge and to grow service development skills in a supportive environment, including simulation training and sessions with a practice development physiotherapist. Continued professional development is encouraged and valued, with regular supervision and in-service training. The team works across extended days and across 7 days.

The physiotherapy department is inclusive and closely collaborates with the local community.

Job Purpose

To provide a specialist rehabilitation, discharge and respiratory physiotherapy service for assessment, diagnosis, treatment and management of patients in the Lewisham and Greenwich NHS Trust.

To participate as a specialist physiotherapist in the emergency department discharge team, including work in the emergency department, ambulatory care, frailty unit and medical admissions unit. This may require working across the Trust and other locations in a range of inpatient, outpatient and community clinical areas.

Job Summary

• To undertake all aspects of clinical duties as an autonomous practitioner.
• To perform advanced physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions, provide a provisional diagnosis and develop an individualised treatment programme.
• To take responsibility for own caseload, within a defined area of the service, with support from a Band 7/8 physiotherapist.
• To take responsibility for own professional learning and contribute to the learning of others in the team as indicated.
• With the team, to participate in evidence-based audit / research activity to further the team's clinical practice and make recommendations for practice changes.
• To supervise junior physiotherapists and physiotherapy students in the clinical setting.

SPECIFIC DUTIES AND RESPONSIBILITIES

• Be professionally and legally accountable for all aspects of your own work and the management of patients in your care.
• Undertake comprehensive, holistic assessments of patients, including those with complex presentations.
• Use manual and other assessment techniques and clinical reasoning to provide accurate diagnoses and identify problems.
• Assess patients’ understanding of treatment proposals, obtain valid informed consent, and work within legal frameworks with patients who lack capacity.
• Develop and deliver effective treatment plans in collaboration with the patient using evidence-based practice.
• Evaluate progress, re-assess, and adjust treatment programmes as required.
• Develop prognoses and liaise with the MDT regarding discharge planning to ensure safe and timely discharge.
• Ensure safe use of gym, electrotherapy, manual handling and respiratory equipment; support Band 5 staff to attain competency.
• Communicate effectively with patients and families, maintaining respect and empathy at all times.
• Participate in weekend and on-call rotas and supervise Band 5 staff when Band 7 staff are unavailable.
• Provide respiratory advice to medical and nursing staff when on the emergency duty rota and on acute ward rotations.
• Supervise and delegate to assistant staff and Band 5 Physiotherapists.
• Work within Trust, departmental and CSP guidelines and stay current with national and local standards.
• When working in community settings, act as a lone practitioner with telephone support from a senior physiotherapist as required; participate in GP outreach clinics as necessary.

• Maintain accurate records in line with Trust and departmental policies for audit and legal purposes.
• Organise, prioritise and plan your own caseload to meet service needs and patient priorities.
• Identify and manage clinical risk within your caseload and area.
• Comply with Trust policies, Health & Safety at Work Act, and Data Protection Act; report complaints, accidents and incidents promptly.
• Be familiar with local fire procedures and all Trust and Therapy emergency procedures; respond as necessary.
• Ensure equipment defects are reported and actions taken; advise carers and staff on safe manual handling and patient management.
• Provide training on equipment and condition management to carers, nursing and other staff when necessary.
• Deputise for the Band 7 in their absence, supervising the operational management of the team.
• Supervise and coordinate students, junior staff and assistants on a day-to-day basis.
• Make appropriate referrals to other health professionals following assessment / need identification.
• Participate in unit and department managerial matters, attend staff meetings, and contribute to induction of new staff.
• Undertake additional duties as required by the team leader or Superintendent in the rotation.

• HCPC Registration
• Degree in Physiotherapy (or equivalent)
• Relevant post-registration experience including core rotations in neuro/MSK/respiratory
• Evidence of attendance at relevant post-registration courses
• MCSP
• Masters or equivalent level study

• Ability to organise and manage own caseload and a problem-solving approach
• Broad NHS experience in an acute trust as a qualified therapist
• Experience of working in the MDT and liaising with other agencies
• Experience of supervising / teaching other staff
• Knowledge of quality issues, clinical governance and outcome measures
• Member of ACPRC or other specialist interest group
• Community experience

• Ability to keep clear, precise records in English
• Ability to write short reports / letters
• Ability to teach one-to-one and in small groups
• Broad understanding of current issues in London healthcare provision
• Service improvement and development skills

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly CRB) to check for any previous criminal convictions.

Salary: £38,682 to £46,580 per year plus HCAS
