Overview

Are you passionate about supporting patients in their own homes and committed to improving their quality of life? We are seeking a Band 6 Specialist Physiotherapist to join the York & SHAR Community Response Team to deliver high standards of holistic and physiotherapy interventions to patients within their home to prevent avoidable admissions to acute care and support discharges. Please note, this vacancy primarily supports the South Hambleton & Ryedale Locality with some expectation to support the York locality as caseload requires. Our established team includes Physiotherapists, Occupational Therapists, Nurses and Support Workers and works with Primary Care and Partners to provide Urgent Care Response, Frailty Hub and short-term reablement/rehabilitation with effective short-term goal setting. This role uses health coaching to engage with the patient and family to ensure expectations are appropriately managed.

Responsibilities
  • Undertake specialist physiotherapy assessment and management of patients referred to the York & SHAR Community Response Service.
  • Determine clinical diagnosis/need and deliver physiotherapy intervention as indicated.
  • Provide support, specialist advice and training to more junior staff and students.
  • Act as a resource within the department and Trust, providing specialist information and advice regarding patients.
  • Maintain records as an autonomous practitioner in accordance with legislation, professional and service policy.
  • Support other colleagues when dealing with this group of patients.
  • Be responsible for a defined area of service working without direct supervision. Clinical work is not routinely evaluated but will be reviewed in line with Trust and service objectives. Access and advice available from senior professional colleagues if required.
  • Initiate evidence-based audits and projects to evaluate and improve clinical practice. Initiate and evaluate user involvement and, where appropriate, make local service changes.

The ability to transport yourself and equipment across the locality during the course of a normal working day is required. Access to a vehicle with business use and a full driving licence is required as the role involves travel to numerous addresses not easily accessed by public transport.
