You will be responsible to provide care to peoplewithin the clinic setting, undertake comprehensive holistic assessment of patients,be able to prioritise clinical need, initiating investigations, determine adifferential diagnosis and implement a plan of care reflecting evidence-basedguidance in conjunction with NICE guidance.


Main duties of the job

Community Falls Team working hours08.30-4.30 Monday - Friday.


Trust adheres to flexible workingpolicy.


You will adopt an MDT approach and undertaketriage within the service determining clinical priority of patients and assess,design, and deliver specialised interventions, exercise and educationprogrammes for patients referred to the Falls service within the clinic setting.

Job responsibilities

  • Towork as an autonomous practitioner within the community Falls Team usingspecialist clinical reasoning skills to providespecialist physiotherapy assessment, diagnosis, treatment and advice to anindependently managed caseload which will incorporate routine and complexpresentation of conditions.
  • Tobegin to develop specialist skills to take a lead role in clinical practiceissues, raising the profile of the service and offering specialist advice toplanning and service development teams to assist in the implementation ofservice improvements.
  • Toconsolidate post-graduate skills and continue professional development,including participation in physiotherapy in-service training programmes andexternal postgraduate training.
  • Todevelop the clinical education of staff within the specialism and beresponsible for the supervision of unqualified and newly qualifiedPhysiotherapists who work under your direct supervision.
  • Participatein research, service development and/or quality improvement activities.

Person Specification
Skills

  • Ability to organise and prioritise complex caseloads.
  • Able to present information, written and orally in a clear and logical manner.
  • Ability to communicate at all levels.
  • Basic IT skills.
  • Able to use own initiative appropriately to enhance service delivery.
  • Presentational skills.

Experience

  • Comprehensive experience at undergraduate and postgraduate level in Falls Assessment Autonomous practitioner responsible assessing, clinical decision making, treatment selection and evaluation of treatment and discharge.
  • Evidence of effective verbal and written communication skills with complex patient presentations.

Qualifications

  • BSc Physiotherapy or equivalent.
  • HCPC State Registration.
  • Continuing Professional Development diary including evidence of reflective practice, training and clinical experience.
  • Basic numeracy and literacy skills to level 1 or equivalent.
  • Basic IT skills.
  • Evidence of post-graduate competence, knowledge and skills in Falls Assessment
  • An understanding of South Tyneside and Sunderland Foundation Trust, its culture, strategic direction, achievements and NHS standing.
  • A proactive ability to represent the profession and the Trust with positive enthusiasm Commitment, motivation and adaptability.
  • Good levels of general fitness Flexibility.

Personal Qualities

  • Ability to carry out moderate to intense physical effort throughout the working day and to work within an
  • environment with competing demands.
  • Ability to maintain high levels of concentration.

  • Driving Licence.
