Community Respiratory Specialist Physiotherapist

An exciting opportunity to work within a well-established Community Pulmonary Rehabilitation Team in Shropshire, Telford and Wrekin, supporting patients to make positive life changes that enhance their health. You will work independently in a community setting, building relationships with the community respiratory nursing teams, local GP surgeries, secondary care respiratory colleagues and inter-disciplinary teams to deliver high-quality care across community settings.

• Provide pulmonary rehabilitation and respiratory physiotherapy to patients in the community in both group and domiciliary settings.
• Triaging new referrals, offering chest clearance advice, post-exacerbation and admission avoidance care.
• Participate in MDT meetings and events.
• Collaborate with community respiratory nursing teams, local GP surgeries, secondary care respiratory colleagues and other community teams to deliver care.

• Registered Physiotherapist with post-registration experience, including working with respiratory patients in the acute or community setting.
• Keen interest in long-term respiratory conditions with excellent respiratory knowledge and skills.
• Desire to further develop skills and knowledge in this field; possess an in-depth clinical knowledge of respiratory conditions, supported by recent experience or qualifications.
• Demonstrate comprehensive assessment skills, knowledge of evidence-based practice, and ability to apply theory to practice with critical appraisal skills.
• Support towards CPD will be provided, including attendance at external courses and conferences as the service allows.
• Excellent communication skills, autonomous caseload management, and the ability to work flexibly within a team.
• Ability to meet the travel requirements of the post.
