The Role

The Community Rehabilitation and Reablement Service provides specialist rehabilitation across East Renfrewshire HSCP. The multi‑disciplinary team consists of physiotherapy, occupational therapy, dietetics, speech and language, nursing, assistant practitioner and rehabilitation support worker intervention. The Service operates over two geographic teams in Eastwood and Barrhead localities and provides supported discharge from Acute settings and coordinated assessment and treatment within an individual’s own home (or other appropriate community or workplace setting) to maximise health gain and independence. Staff work as part of a flexible and responsive team which aims to provide a single point of access to assessment of need followed by time‑limited and appropriate rehabilitation. Close collaboration exists with service users, carers, primary and secondary healthcare, social work services and the voluntary sector.


Duration, Location, and Working Pattern

  • Location‑based in Eastwood Health and Care Centre
  • Permanent Contract
  • Part‑time position (22.2 hours per week)
  • Shift pattern for this post are 3 out of 7 days
  • Please note the salary is Pro Rata for part time hours.

Key Responsibilities

  • Undertake a comprehensive assessment of patients (including screening and multi‑professional baseline assessments) with a complex presentation using investigative skills, analysing clinical and non‑clinical information (i.e. social information, environmental) and to provide a functional diagnosis for individual patients to determine their need for Physiotherapy intervention, using formal assessment tools as appropriate.
  • Following assessment, act autonomously to assess, develop, implement and evaluate highly specialised Physiotherapy interventions/therapy programmes for a range of patients. This will include patients who have highly complex presentations and those with multi‑factorial health and social needs, requiring multi‑service/multi‑agency input. Incorporate risk assessment to maximise functional independence and rehabilitation potential within a specialist area.
  • Provide highly specialist interventions based on a problem solving and advanced clinical reasoning approach. Deliver specialist interventions using a wide range of treatment techniques. Monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention.
  • Assess for a range of equipment, e.g. orthotics and walking aids, providing appropriate equipment and referral onto other specialist services when required, e.g. Orthotics, Community OT, Equipu, Westmarc and Housing Dept. To be responsible for the care, advice regarding maintenance and issue of equipment.
  • Demonstrate professional expertise within the speciality.
  • Actively lead identified areas within clinical effectiveness (such as audits, journal clubs and projects) to support the Physiotherapy health care governance agenda. Assist / cooperate in all matters necessary for the efficient management and development of the Rehabilitation Service in accordance with Service Policy.
  • Induct rotational Physiotherapy staff and students and participate in the induction of new team members.

Knowledge, training, qualifications and/or experience required to do the job
Essential

  • The post holder will hold a Degree/Diploma or equivalent in Physiotherapy
  • The post holder will be registered with the relevant Health Professions Council e.g. HCPC
  • Minimum of at least 3-5 years at Band 6 level with Postgraduate Diploma level or equivalent specialist experience which should include relevant specialist experience and/or attendance at training relevant to working within the following; Adult Rehabilitation, Neurology, Orthopaedics, Respiratory, Medicine or Care of the Elderly Specialities.
  • Excellent communication skills.
  • Should hold a current full driving licence.

Desirable

  • Postgraduate Masters level study relevant to clinical area
  • Leadership skills
  • Knowledge of IT systems.

From 1 April 2026, the Agenda for Change full‑time working week will reduce from 37 hours to 36 hours. Part‑time working hours will be reduced on a pro‑rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
