Advanced Clinical Practitioner Physiotherapist - Ageing Well Team

Woodsley and Holt Park Primary Care Network covers a large geographical area in Leeds with a diverse population. There are 10 practices within the network with a population of over 100,000 patients.


Primary Care Networks are groups of general practices working together with a range of local providers, across primary care, community services, social care and the voluntary sector, to offering more personalised, coordinated health and social care to their local populations. Main duties of the job

Key Responsibilities:


Clinical Assessment and Management


To deliver and participate in the clinical care of patients with frailty conditions. Prioritise health needs and intervene appropriately.


Conduct comprehensive, holistic assessments of patients with frailty and dementia, identifying physical, functional, and psychosocial needs. Develop and implement individualised treatment plans focused on maintaining or improving mobility, strength, balance, and overall functional ability.


Order and interpret investigations, laboratory tests, x-rays and act on results as appropriate.


Be an Independent Prescriber and initiate drug therapy/medications as per legal requirements for the practitioner’s professional body, national and local guidelines.


To be professionally and legally accountable for all work undertaken. Practice at an advanced level of professional autonomy and accountability that is within the PCN policies and professional code of conduct (NMC, HCPC).


Ensure that clinical documentation is comprehensive, accurate and contemporaneous.


Ability to adapt specialist clinical knowledge and skills to different clinical settings and influence service delivery and patient care.


Utilise and demonstrate appropriate communication styles, to ensure patients are fully informed and consent to treatment.

Role Summary:


We are seeking an experienced and highly motivated Advanced Clinical Practitioner Physiotherapist to join our Ageing Well Team, based within Woodsley and Holt Park Primary Care Network. The role focuses on delivering physiotherapy interventions within a proactive care framework aimed at preventing deterioration, promoting independence, and enhancing quality of life for older adults living with frailty and/or dementia. The successful candidate will work collaboratively across multi-disciplinary teams to support complex patient needs through comprehensive assessment, treatment, and holistic care planning.

Qualifications

  • Essential Qualifications and Experience:
  • Registered Physiotherapist with HCPC registration.
  • Advanced Clinical Practitioner qualification or equivalent advanced clinical training.
  • Extensive experience working with older adults, particularly those with frailty and dementia, in primary or community care settings.
  • Strong knowledge of frailty syndromes, dementia care pathways, and relevant clinical guidelines.
  • Experience in multidisciplinary team working and integrated care delivery.
  • Excellent communication and interpersonal skills, with the ability to engage patients and carers effectively.
  • Experience of completing Advanced Care Planning with patients and their families/carers, including completion of ReSPECT forms.
  • Prescribing qualification (Independent or Supplementary).
  • To have experience of working with a range of different clinical conditions. Some expertise of MSK and neurological conditions would be of benefit.
  • Experience in service development and quality improvement.
  • Competence in using clinical IT systems and digital health tools.
