Locum Physiotherapist - Discharge Service Team in Islington UK earning 31.77 per hour Hourly Ongoing and Full Time Embark on an exciting career journey as a Locum Physiotherapist with the Discharge Service Team in vibrant Islington. - Support residents in risk assessment and intervention design within the hospital pathway.- Contribute to the Discharge to Assess pathway through referral documentation screening and planning.- Perform post-discharge assessments in residents' homes, providing therapy interventions to support recovery and independence.- Consider environmental factors, provision of equipment, minor adaptations, and manual handling to optimise resident care.- Collaborate with residents on discharge pathways to create Reablement Plans and Goals, focusing on strength-based practice.- Embrace flexible working arrangements with a mix of home and office-based tasks.Requirements:
