This post is set within a multidisciplinary team working across acute and community supporting those with frailty to maximise their independence and wellbeing within their local communities, supporting early discharge and preventing avoidable hospital admissions.

We welcome applications from experienced and innovative practitioners with an appetite for service improvement and self-development. This post will work across the primary secondary interface, and join the HFRS team aligned within acute front doors in Renfrewshire City HSCP areas. Critical to the success of these roles will be facilitating and enabling communication between and within multi-disciplinary teams across acute and community services.

Duration, Location, and Working Pattern:

  • This is an exciting opportunity for an experienced Physiotherapist within Renfrewshire HSCP at Renfrewshire House
  • The post is full-time although part time options may be considered provide resilience to the service
  • The shift pattern for this post is five days out of seven
  • These posts will be peripatetic working across acute and community to support the development of the 7 day service

Key responsibilities:

  • Undertake a comprehensive assessment of frailty positive patients (including screening and multi-professional baseline assessments) with a complex presentation using investigative skills, analysing clinical and non clinical information (i.e. social information, environmental) and to provide a functional diagnosis for individual patients to determine their need for Physiotherapy intervention, using formal assessment tools as appropriate
  • Following assessment, act autonomously to assess, develop, implement and evaluate Physiotherapy interventions for a range of patients. This will include patients who have highly complex presentations and those with multi-factorial health and social needs, requiring multi-service/multi-agency input. Incorporate risk assessment to maximise functional independence and rehabilitation potential within a specialist area
  • Provide specialist interventions based on a problem solving and advanced clinical reasoning approach. Deliver specialist interventions using a wide range of treatment techniques. Monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention. i.e. frailty specific interventions, therapeutic handling techniques, functional rehabilitation, adaptation, specialist equipment provision and use, education and health improvement. Education and training will be aligned to the local induction and competency frameworks.

From 1 April 2026, the Agenda for Change full-time working week will reduce from 37 hours to 36 hours. Part-time working hours will be reduced on a pro-rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
