Community Neurological Rehabilitation Service - High Weald Lewes and Havens

Permanent post for up to 30 hours per week (less hours considered).


The Community Neurological Service (CNRS) based at Newhaven Polyclinic and covering the High Weald, Lewes and Havens area (HWLH) is seeking to employ an experienced Physiotherapist to join the team.


This is a clinical role and you will be expected to undertake highly specialist physiotherapy assessments and provide treatment programmes for neurologically impaired patients in the community setting.


The service working hours are Monday – Friday 8:00 – 17:00.


Due to the domiciliary nature of this post, a full UK driving licence and access to a car is essential.


Key Responsibilities

  • To provide high level clinical expertise and decision‑making using evidence‑based methods regarding patient care.
  • To provide timely specialist advice and support to patients, their relatives and carer’s, other SCFT staff and colleagues in primary/secondary and third sector organisations.
  • To support Band 6 clinicians with complex case management, caseload management, prioritisation skills and delegation skills.
  • To work alongside other leads to manage the support staff workloads, integrating skill sharing initiatives.
  • To support undergraduate physiotherapy student placements and support the Band 6 staff supervising the students.
  • To provide performance information to service managers to support the delivery of KPIs, management of demand and capacity and to inform future workforce, skill mix requirements and service development.
  • To report incidents and manage incidents and complaints to reduce risk. To share learning from incidents and risk with the team and wider organisation as appropriate.

Additional Responsibilities

  • To retain own clinical competency to carry out assessment, treatment, rehab interventions and to contribute to timely discharge where appropriate, particularly for highly complex patients.
  • To use advanced knowledge, skills and expertise to facilitate and drive future service development.
  • To work with the AHP professional leadership Team to ensure delivery of evidence-based clinical practice.
  • To facilitate the delivery of a robust clinical supervision structure in line with Trust policy.
  • To ensure all staff work to appropriate competency frameworks and manage performance of individuals through the appraisal and supervision process.
  • To work collaboratively with other leads to facilitate effective operational management of allocated services.
  • To ensure the service functions within financial constraints through collaborative working with the budget holders, and to contribute to the annual business planning process.
  • To ensure effective delegation as per Trust guidelines and to support the implementation of the Calderdale framework and skill sharing initiatives.
  • To manage own caseload of complex cases within defined clinical area.
