Clinical Lead Physiotherapist – Integrated Community Therapy and Rehabilitation Service, North Lancs


Overview

We have an exciting opportunity for a dynamic, experienced physiotherapist to fulfil the role of Clinical Lead Physiotherapist within the Integrated Community Therapy and Rehabilitation Service – North Lancs.


This is a permanent contract. Working alongside the existing Clinical Lead Physiotherapy and Occupational Therapy posts, this role will include operational leadership and management responsibilities to support a large community therapy team over a range of therapy pathways – from urgent response ‘Home Assessment for next day D/C’ up to 3‑day to 10‑week community therapy response, delivering falls prevention, addressing complex Continuing Health Care needs and responding to resident needs in nursing care home beds.


The post is temporarily based at the Royal Lancaster Infirmary, central Lancaster, while awaiting further information regarding a permanent team base. The post holder will manage a caseload of complex community patients and liaise with other local hospital and community‑based services across primary, secondary, community and local authority adult social care. Experience of highly effective multi‑disciplinary team working is an essential leadership requirement.


The post holder will support the training and development of therapy colleagues, work alongside other Clinical Lead Therapy colleagues, and manage the deployment and supervision of an integrated therapy team within a defined clinical area/geographical community.


Key Responsibilities

  • Provide operational leadership and management to support service delivery and improvement.
  • Deliver falls prevention, complex Continuing Health Care and nursing home resident care.
  • Support training and development of therapy colleagues.
  • Deploy and supervise an integrated therapy team within the defined community.
  • Manage daily service delivery, including waiting lists, HR issues, appraisals, probation reports, risk assessments, performance, E‑Roster, incident reports and compliance with Care Quality Commission Standards.
  • Collaborate with Clinical Team Leaders and stakeholders to ensure coordinated, multidisciplinary and multi‑agency service delivery.
  • Plan, deliver, evaluate and develop services within the clinical area.
  • Provide therapy assessment and treatment to patients with diverse health needs, including complex or chronic conditions.
  • Monitor and analyze performance data against agreed targets for the team.
  • Act as an effective team player, using initiative, empathy and excellent communication.
  • Embrace innovation and integrated working, committing to best practice for patient‑centred care.
  • Demonstrate a commitment to clinical leadership.
