Key Responsibilities
  • Conduct assessments and develop treatment plans for patients in intermediate care beds.
  • Facilitate patient discharges, ensuring smooth transitions within the community setting.
  • Assist with the provision and management of equipment, ensuring safety and compliance with manual handling guidelines.
  • Supervise and mentor junior staff, guiding their professional development.
  • Drive within the community, carrying and delivering necessary equipment.
  • Apply your knowledge to manage conditions such as arthritis, heart disease, diabetes, dementia, MSK, COPD, and falls effectively.
