We are looking for a dynamic SpecialistPhysiotherapist to join our Community Therapy Team in Darlington. The post holder will work across a range ofcommunity areas including Falls, Rehab beds, Amputees, Stroke and Neuro in andaround Darlington including patients homes, care homes, our intermediate carerehab beds and within our rehabilitation unit at Hundens Lane.

Main duties of the job

The post holder will independently manage a caseload thatincludes some highly complex patients and provide specialist assessments andtreatment plans to adults referred to the team who present with a range ofphysical dysfunction and/or disabilities resulting from injury, illness,surgery or chronic disease, within a range of community settings. They willwork alongside Physiotherapists, Assistant Practitioners and Speech andLanguage Therapists within the community therapy team and other professionalswithin the wider organisation and partner organisations.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities

Organise andprioritise own workload and to monitor those you clinically supervise within adefined area.

- Independently screen, prioritise and plan interventionsfor routine and priority referrals received within community therapy services.

- Use sound clinical judgement to prioritise patients withina demanding and unpredictable environment ensuring effective use of resources

- Assist staff under your supervision to prioritise theircaseloads and advise them on appropriate interventions in response to complexcases on a daily basis.

- Undertake holisitic and comprehensive specialistassessments of patients including those with diverse or complexpresentations/multi pathologies that change across the caseload.

- Utilise analytical and clinical reasoning skills andmanual assessment techniques to provide accurate diagnosis of their condition,these skills may be advanced; intepret facts and select the appropriatetreatment and implement rehabilitation programmes.

- Work with the multi-disciplinary team tofacilitate patient centred goals .

  • Ability to travel and carry small items of equipment on a daily basis to fulfil the requirements of the post
  • Ability to meet physical demands of the post

Qualifications

  • BSc/MSc in Physiotherapy
  • Evidence of CPD, including attendance at recent post-graduate courses relevant to the clinical fields in the specialism
  • An understanding of County Durham and Darlington NHS Foundation Trust, its culture, strategic direction, achievements and NHS standing
  • Involvement in specific interest groups

Statutory Registration

  • HCPC
  • Member of CSP

  • Comprehensive and relevant qualified experience in physical rehabilitation including recent general rehabilitation experience

  • Experience in working in a variety of settings
