Position Overview

We are looking for a motivated Band 6 Specialist Physiotherapist to join the Harrow Integrated Care Team, based in Stanmore. You will deliver a high quality, community based physiotherapy service, managing your own caseload and working autonomously in line with professional standards and evidence based practice.


As part of a multidisciplinary team, you will build strong relationships with referrers, support coordinated patient care, and contribute to service delivery through the supervision of junior staff, rehabilitation assistants and students. You will also actively engage in continuing professional development, audit and service improvement to support both your own growth and the ongoing development of the service.


Responsibilities

  • Undertake specialised uni professional and multi professional assessments for clients with complex needs
  • Develop, implement and review individualised treatment and rehabilitation plans for clients presenting with frailty related conditions and multiple co morbidities, including neurological, musculoskeletal, osteoporosis, learning disability and mental health conditions
  • Participate in the assessment process as required, completing holistic overview assessments and complex specialist assessments, including the identification and ordering of appropriate moving and handling equipment to support rehabilitation needs
  • Communicate relevant medical information, assessment findings, advice and recommendations effectively with clients and across partner agencies, in line with Trust guidelines on information sharing and informed consent
  • Work collaboratively with a range of agencies, including GPs, Social Services, healthcare professionals, voluntary organisations and residential care providers
  • Obtain valid informed consent for assessment, treatment and information sharing, applying legal and professional frameworks appropriately, including in situations where clients lack capacity
  • Maintain up to date knowledge of relevant clinical guidelines and, with supervision, apply these effectively to support evidence based clinical practice
