We have a fantastic opportunity for an enthusiastic band 6 or an experienced band 5 ready to step up to join our dynamic Wyre Forest Glades Neighbourhood Team based at Kidderminster Health Centre.


You will work as part of a supportive multidisciplinary team, enabling people to stay well at home through admission prevention, facilitating discharges and urgent community response work as well as routine community physiotherapy referrals.


You will work within a friendly, well established multidisciplinary team including Physiotherapists, Occupational Therapists, Nurses, Healthcare Assistants, Therapy Assistants, GPs and Social Workers. The caseload is varied and rewarding, involving holistic falls assessments and rehabilitation, orthopaedic and neurological conditions, frailty, respiratory and musculoskeletal presentations, palliative and end of life care, and urgent community needs such as equipment provision, sudden deterioration, manual handling challenges and carer breakdown.


This is a full time post, but we would consider applicants looking for part time.


The Neighbourhood Team operates seven days per week between 7.30am and 18.30pm, while the Therapists work Monday to Friday between 8.00 and 17.00, with bank holiday cover on a rota basis. Flexible working available.


Main duties of the job

Working as an autonomous practitioner within an integrated team you will provide timely and expert professional assessment and treatment interventions to patients in their own homes. This will be achieved through a combination of hands‑on expert clinical skills and knowledge as a Physiotherapist to treat, educate and support patients and by working with patients and their families to assess, plan and implement appropriate care plans and treatment.


Provide case management for a designated caseload of patients who have immediate and/or complex care needs.


Triage patients to ensure prioritisation of clinical need.


Develop and co‑coordinate individualised rehabilitation plans in close liaison with the multi‑disciplinary team, the patient and their families as well as all other services.


Work holistically with all team members, carrying out joint visits and actively participating in the integration of services by increasing the knowledge of all team members through case discussion and collaborative working.


During the induction the team will help you feel confident in your role and in how the wider MDT works within the community, and you will have access to regular supervision and training as well as your annual appraisal.

Person Specification
Experience

  • A minimum of 2 years post graduate experience including work in a variety of specialities
  • Experience of supervision of junior staff or students.
  • Experience of delegation to qualified/unqualified staff
  • Recent experience of working in a community setting
  • Experience of working in an integrated team
  • Experience within relevant clinical field.
  • Previous experience at Band 6
  • Experience of implementing change in clinical practice/ audit

Knowledge

  • A range of evidence-based Physiotherapy clinical skills and knowledge appropriate to the role
  • Knowledge of the roles and responsibilities of the professional groups within an integrated team
  • A good level of knowledge in a wide range of physical and mental health conditions
  • General understanding of typical medications used to treat a variety of conditions
  • Knowledge of research and audit processes

Additional Criteria

  • Able to meet the travel requirements of the role

Qualifications

  • Diploma / Degree in Physiotherapy.
  • HPC registration.
  • Evidence of relevant post Graduate courses / CPD maintained in portfolio.
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
  • Evidence of relevant validated clinical courses
  • Clinical Educators Qualification
