Senior Physiotherapist (Intermediate Care) - part time (20 hours per week) six month secondment. Based at Princess of Wales Hospital, Ely, working as part of the discharge to assess pathway. Closing date: 11 February 2026.

We are a dynamic team within Cambridgeshire and Peterborough NHS Foundation Trust, providing clinical therapy support to an expanding service based in Cambridgeshire and Ely, supporting patients across the South locality. The post holder will ensure patients receive the best possible care in their own homes, maximising potential in the initial phases on discharge from hospital in our short stay service.

Successful applicants will assist in developing integrated care workers to support clinical delivery and will work proactively with the wider multidisciplinary team to ensure patient flow and safety. The service aims to provide outstanding, patient-focused, community-based health care and therapy, optimising people’s potential to achieve their goals by supporting earlier discharge from hospital.

Main duties of the job

The role involves providing clinical therapy support to the expanding service based in the North, supporting patients across the Cambridgeshire and Ely locality. The post holder will ensure patients receive the best care in their own homes, maximise patients’ potential in the initial phases on discharge from hospital in our short stay service, and provide therapy within our interim bed environment while supporting the prevention of hospital admissions. Successful applicants will assist in the development of integrated care workers to support the clinical delivery of the service and work with the wider multidisciplinary team to ensure patient flow and safety. The vision remains to provide outstanding, patient-focused, community-based health care and therapy, optimising patients’ potential to achieve their goals by supporting earlier discharge from hospital; early assessment for care needs and avoidance of hospital admission.

Please note: For this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. Job responsibilities

  • To be professionally and legally accountable for all aspects of your own and delegated work
  • To work within codes of practice and professional guidelines
  • To undertake holistic assessment (including those with complex presentations and multi-pathologies) making use of specialist clinical reasoning skills
  • To use evidence-based practice, to ensure clinical care is effective and appropriate
  • To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure patients receive appropriate care in the most suitable setting
  • Organise and manage own time, delegating work appropriately
  • Conduct risk assessments and health and safety assessments, and report any changes or newly identified risks
  • To demonstrate the importance of gaining patient and carer consent in all interventions
  • To effectively communicate verbally with patients, carers and colleagues, including capable communication with those with hearing loss, cognitive impairment, mental health issues, or other communication barriers
  • Actively involved in the development of the service/organisational aims and objectives of the Teams and the new model of care
  • To work effectively within the team structure and liaise with MDT members and other agencies by attending MDT meetings and case reviews
  • To ensure accurate and timely written records in line with Trust policy and HCPC guidance, reporting on issues as appropriate
  • To provide specialist advice to MDT members, service users and others on changes to care plans and patient progress
  • To support day-to-day planning and delivery of high-quality care

Person SpecificationEducation / Qualifications
  • Degree or equivalent (BSc, MSc, or Diploma) entry-level qualification into Physiotherapy
  • Current HCPC registration
  • Documented evidence of continuing professional development
  • Willingness to undertake Fieldwork/Practice Educator qualification
  • Member of CSP

  • Relevant post-graduate clinical experience in varied settings
