Overview

An exciting opportunity has arisen for an enthusiastic and innovative Physiotherapist to join the therapy team at Longmoor House. Longmoor House is a newly refurbished Integrated Care Unit that provides step‑down care from acute trusts and step‑up care from the community for medically optimised patients who are anticipated to have ongoing nursing, therapy and reablement needs. The service is based on a nursing and therapy led reablement model with advanced nurse practitioner and GP cover, utilising a holistic and person‑centred approach to support individuals requiring our services.


Responsibilities

  • Work in a supportive and caring environment.
  • Develop professionally and deliver great healthcare to make a positive difference in patients’ lives.
  • Work within an integrated team with all members of the MDT and partner services.
  • Participate in quality initiatives to improve patient experience and care.
  • Provide a rehabilitation service as part of a multidisciplinary team within a sub‑acute intermediate care setting.
  • Work autonomously and take responsibility for a caseload, ensuring evidence‑based, individualised and effective treatment is administered with the outcome of a timely and safe discharge.
  • Demonstrate excellent time management, clinical reasoning and risk assessment skills.
  • Receive support from senior staff and team lead with regular supervision and in‑service training.
  • Work on a 7‑day per week rota.
  • Effectively manage a caseload of patients’ comorbidities as an autonomous practitioner, using evidence‑based practice and patient‑centred principles to assess, plan, implement and evaluate interventions.
  • Provide leadership for junior staff through supervision and appraisal.
  • Plan, develop and evaluate the Physiotherapy service within a designated area/team, holding responsibility for defined projects.
  • Take a holistic approach in the assessment and treatment of patients, providing appropriate, high‑quality care to a defined caseload which may be of a complex nature.
  • Work in partnership with other professionals to enable patients to be maintained in an appropriate environment, thereby avoiding acute hospital admission.
  • Promote effective teamwork, delegating activities appropriately for the benefit of patients and service delivery.
  • Contribute to the maintenance and development of the Physiotherapy profession within the team and facilitate effective learning for all students and practitioners.
