Across Gloucestershire we have had major investment in our Home First and Reablement Team and, as a result, we have an opportunity for a Band 6 Physiotherapist to join our Team and be a key part of developing our therapy‑led service in the South Cotswolds. Our Team is based in Cirencester and works closely with the GP practices in the Primary Care Network, alongside our Adult Social Care colleagues and voluntary organisations in the locality.

We can offer you the opportunity to work in a very supportive integrated team, with access to training, regular supervision, annual appraisal and the autonomy to be a lead professional in our new service delivery. As you will be driving around the beautiful Cotswolds delivering our service, being a car driver is essential to the role. Main Duties of the Job

In line with the Government hospital discharge and community support policy, and the discharge to assess model, our Home First service is a time‑limited assessment service for people in their own homes to facilitate rapid discharge from Acute and Community hospitals. We also provide Reablement to patients in the community, to prevent hospital admission and to facilitate maximum independence, working on jointly agreed goals, promoting personalised care.

Job Responsibilities

As a Physiotherapist you will be a key part of our team, providing assessment, goal planning and reviews, working alongside Occupational Therapists, Nurses, Reablement workers and rehabilitation assistants to enable our patients to gain their maximum level of independence in their own home. The caseload will be varied, ranging from rapid assessment to prevent hospital admission, Home First assessments to establish pathways of care following hospital admission, through to setting goals as part of a reablement pathway.

Person Specification

Essential Qualifications

  • Diploma / Degree in Physiotherapy
  • HCPC registration
  • Driving licence and use of vehicle

Desirable Qualifications

  • Membership to a relevant professional body e.g. CSP

Essential Experience – Length and/or Nature

  • Broad range of clinical experience across a variety of clinical areas
  • Experience of managing junior members of staff and rehabilitation assistants
  • Evidence of working unsupervised
  • Evidence of working across multi-disciplinary and multi-agency boundaries

Essential Professional / Managerial / Specialist Knowledge

  • Able to deliver community rehabilitation and treatment of patients with complex clinical presentations
  • Demonstrates knowledge of Home First pathway

Desirable Professional / Managerial / Specialist Knowledge

  • Experience of performance management
