An opportunity has arisen in the complex assessment beds therapy team (Pathway 3) in North Somerset for a motivated, compassionate and ambitious Physiotherapist who has a genuine interest in working in rehabilitation.

You will be part of a therapy team responsible for the management of service users who have been recently discharged from an acute hospital setting and are undertaking a period of assessment and rehabilitation in order to establish their long term place of residence. Your role will include the assessment and treatment of service users and delivery of rehabilitation programmes.

This role will be based within the Pathway 3 team which covers various nursing homes across North Somerset, the role requires the flexibility to work across the Pathway 3 bed bases.

Currently the service is Monday-Friday however will potentially expand to cover weekends in future. We are also able to support flexible working patterns.

Responsibilities
  • Responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients with complex needs often arising from the frail elderly cohort
  • Use clinical reasoning skills to assess, plan, implement and evaluate, patient- centred intervention programmes incorporating physical, psychological, social, leisure an employment needs, using an evidence-based practice approach, promoting high quality and harm free care
  • Work in partnership with the wider multi-disciplinary team including primary and secondary care, social care, voluntary agencies and others according to service users needs
  • Collaborate with service users, carers and family in identifying appropriate and achievable goals as part of the overall care plan, which will maximise quality of life and independence
  • Provide a service that focuses on achieving and demonstrating overall improvements in health care for patients who are complex, frail and multiple long-term conditions
  • Facilitate referrals to other services or investigations based on your assessment
  • Take responsibility for the development of other members of the team including peers, junior staff, and students whilst assisting other junior staff to organise, prioritise and manage their caseload
  • Participate in training; this may involve assisting in the delivery of training both within the service and to other health professional as appropriate
  • Operational management of a delegated number of staff with support from senior manager

Qualifications Essential

  • BSc or equivalent in Physiotherapy
  • HCPC registered

Skills, Experience, Knowledge Essential

  • Effective communication skills
  • Ability to function effectively as a team member
  • Ability to work autonomously and know when to seek appropriate advice
  • Ability to make clinically reasoned judgements
  • Ability to adapt communication for individuals with communication difficulties
  • Ability to reflect and critically appraise own performance
  • Competent IT skills
  • Recent relevant experience within the community setting including management of the frail complex patient
