Job Overview

An exciting opportunity has arisen to join the Critical Care team as a Follow Up Physiotherapist, undertaking a lead physiotherapy role in assessing, planning and evaluating patients needs through the Critical Care Follow Up clinic.


You will be part of a multidisciplinary team within the critical care unit, providing advice to medical, psychological and nursing staff.


Main Duties

  • To provide a specialist rehabilitation service to support the most complex of patients. Ensuring both short and long term rehabilitation needs are addressed. Ensuring the appropriate interventions, guidance, or support is offered at the correct time. Ensuring parents and their families are informed and included in their rehabilitation journey, and are offered the correct information.
  • To perform specialist comprehensive therapeutic assessments to identify rehabilitation needs and provide a patient centred individualised rehabilitation programme ensuring continuity of care and co-ordination of the patient rehabilitation process. Working with complex facts or situations requiring analysis, interpretation, comparison of range of options.
  • To ensure appropriate early referrals are made to enable the patient to receive the best possible service(s) available for their rehabilitation needs.
  • To provide a post critical care follow up service. Ensuring the correct patients receive the appropriate physical, psychological and financial services to aid ongoing recovery and improve their quality of life.
  • To be fully integrated into the team ensuring excellent communication at all levels.
  • Assess patient understanding of treatment proposals, gain valid formal consent and jointly agreed goals.

Knowledge
Essential criteria

  • Specialist knowledge of rehabilitation pathways
  • Knowledge and understanding of current EBP of Critical Illness and recovery
  • Knowledge of NICE guideline and National Service Framework appropriate to specialist field

Desirable criteria

  • Mental Health conditions, working knowledge of ICU delirium and its treatment
  • Knowledge of cognitive assessments which are compatible with patients post critical illness

Experience
Essential criteria

  • Acute inpatient experience
  • Evidence of ability to work in multi‑disciplinary and multi‑agency environment

Desirable criteria

  • Experience of working in a critical care environment

Training
Essential criteria

  • Evidence of recent post‑graduate learning and education in subjects relevant to area of practice

Desirable criteria

  • Advanced neurological therapeutic skills
