2. Provide a quality driven, evidence based physiotherapy rehabilitation in the community setting, providing physiotherapy assessment, diagnosis and treatment plans for patients requiring physiotherapy.
   
3. Be a specialist physiotherapist and have a detailed professional and clinical knowledge. The post holder will be a specialist physiotherapist practitioner.
   
4. Be skilled in the assessment of complex/specialist physiotherapy care/support needs and in the development, implementation and evaluation of care/support packages for patients and families.

2. Interdisciplinary working with other health professionals for holistic assessments and treatment plans including assessment, goal setting, planning and developing therapy treatment programmes; monitor and review, as part of the Physiotherapy component of the D2A Pathway 1 Service.
   
3. To write and review treatment plans to enable patients to maintain maximum independence.
   
4. To undertake treatment plans identified by physiotherapists.
   
5. Act at all times in a manner which respects privacy, dignity and confidentiality.
   
6. To maintain patients notes in accordance with Trust policy.
   
7. To manage own caseload, time management and diary arrangements.
   
8. To manage, supervise and coordinate more junior physiotherapy staff. Planning and delegating their workload as appropriate. Provide guidance and delegated tasks to support workers conducive to the individual rehabilitation programme delivered within the Home First Team.
   
9. Triage referrals to determine appropriate level of physiotherapy input required to meet individual patient needs.
   
10. Support those who require rehabilitation assessment and treatment, to prevent hospital admission and maximise functional independence.
    
11. Support those who have been referred from hospital to discharge to assess pathway 1 for assessment and rehabilitation within their own home or in reaching in order to avoid an acute admission into Community Hospital bed.
    
12. To advise and communicate effectively with patients (who frequently have problems with understanding), carers and other members of the multidisciplinary team, to maximise rehabilitation potential.
    
13. Monitor the individuals progress and amend the aims and goals of treatment accordingly. Communicate effective complex information to members of the Multidisciplinary Team.

2. Qualifications: Degree in Physiotherapy.
   
3. Experience: Relevant experience to the role including assessment and management of various conditions within the area of clinical speciality; Have extensive skills and knowledge to provide assessment and treatment to patients with multiple long term and sub-acute health conditions.
   
4. Skills: A sound knowledge of the management of physiotherapy especial frail elderly patients. Ability to assess, diagnose and treat patients; Demonstrate detailed understanding of clinical competence required in the Discharge 2 Assess in Pathway 1 and admission avoidance; Good working knowledge of Physiotherapy Care Pathways and their objectives.
   

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
