What you will do:

  • Deliver therapeutic interventions for patients on the ICT pathway 1
  • Equip patients with the necessary medical equipment required for their progress
  • Monitor and review patient progress to ensure optimal recovery
  • Support the development of Integrated Care Work (ICW) processes and procedures
  • Enhance patient flow to streamline healthcare delivery
  • Provide dedicated support to all patients on step-up and step-down pathways

Requirements:

  • Registered with the Health and Care Professions Council (HCPC)
  • Minimum of 2 years of experience in a relevant setting
  • Car driver preferable to maneuver efficiently across our county-wide facilities
