What you will do

  • Provide expert assessment and intervention for patients with neuro-related issues within the community.
  • Develop, implement, and review personalised rehabilitation programmes.
  • Liaise with other healthcare professionals for a holistic approach to patient care.
  • Conduct home visits and assessments, utilising your own vehicle to reach patients across the Huddersfield and Halifax area.
  • Maintain detailed patient records and communicate progress effectively with the wider team.
