Social Worker - Adults Multi-Disciplinary Team

Join our team as a Locum Band 5/6 Physiotherapist within the Acute Team in Sutton Coldfield, UK, earning an attractive £21 to £26 per hour on an Ongoing basis. This full-time opportunity allows you to work from Monday to Friday, giving you the chance to make a real difference in an acute hospital setting. If you have a knack for assessment, treatment planning, implementation, and discharge planning/facilitation for general hospital patients, then we want to hear from you! We are open to considering Band 5 candidates depending on their experience, especially those familiar with HCOP and General Medicine, as well as areas like admission avoidance, Trauma & Orthopaedics, Stroke, and Rehab.

What you will do

  • Undertake comprehensive assessments of patients to develop personalised treatment plans aimed at improving function and mobility.
  • Implement and evaluate treatment plans to facilitate recovery, maintaining patient progress and well-being.
  • Plan and facilitate patient discharge ensuring a smooth transition from hospital to home or other care settings.
  • Help maintain efficient patient flow within the hospital to ensure timely care for all patients.
  • Work collaboratively within a multidisciplinary team to provide holistic care to patients.
  • Participate in routine acute hospital work, gaining experience across mainly HCOP, General Medicine, admission avoidance, Trauma & Orthopaedics, Stroke, and Rehab areas.
