If you are passionate about delivering exceptional community-based therapy, preventing unnecessary hospital admissions and improving quality of life for service-users whilst being committed to rehabilitation in a home environment, we'd love to hear from you!


We'll give you the opportunity to develop specialist community skills, enhance your multidisciplinary working and develop key leadership skills.


The Sirona Integrated Neighbourhood Teams (INT) are an innovative and dynamic service. We provide active rehabilitation to patients in their place of residence to maximise their functional potential and support them to remain independent. Our therapists support hospital discharges, admission avoidance and long term self‑management, enabling patients to live better lives in their home environment.


You will be working within a friendly and supportive INT alongside other Therapists, Assistant Practitioners, Nurses, Nurse Associates, Advanced Clinical Practitioners, Support Workers and Administrators.


We can support some flexible working patterns - currently this service runs from 8am - 8pm - 7 days a week. Enhanced pay for weekends & bank holidays, mileage is reimbursed whilst you carry out your duties at work.



  • Initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs.
  • Use clinical reasoning skills to assess, plan, implement and evaluate, patient‑centred intervention programmes using an evidence‑based practice approach, promoting high quality and harm free care
  • Responsible for proactive service and own caseload management
  • Responsible for allocation of work and efficient service/team coordination
  • Develop and maintain strong, communicative working relationships to prevent avoidable admissions and facilitate timely discharges.
  • Collaborate and negotiate with service users, carers and family in identifying achievable goals as part of the overall care plan to maximise quality of life and independence.
  • Facilitate appropriate referrals to other services based on your assessment
  • Assess and arrange the provision of equipment within scope of practice; reporting any issues
  • Take responsibility for the development of other members of the INT including peers, junior staff, and students
  • Maintain accurate, informative and legal records
  • Actively participate in training; this may involve assisting in the delivery
  • Identify risks and act accordingly to minimise risk to self, colleagues and service users
