Specialist Band 6, Physiotherapist – Community Stroke, Southwest Lincolnshire. This post has the potential to offer rotations/experience within other Neuro services across Lincolnshire. The Lincolnshire Stroke Service (LSS) provides a pathway from acute inpatient settings to community delivery, with the acute element centralised at Lincoln County Hospital and the community arm delivered countywide from four bases. The post is hosted by Lincolnshire Community Health Services (LCHS) and requires the ability to travel. The post may include rotations between other neuro services and involvement in weekend therapy cover. A full driving licence and access to a vehicle are essential.

• To provide the specialist physiotherapy element in the Lincolnshire Stroke Service, including clinical specialist support, professional advice and practical guidance across Acute and Community stroke pathways.
  
• To act as case manager to a designated caseload, ensuring all patients access the elements of their care plan and that individual care plans are delivered and progressed by the team.
  
• To manage a clinical caseload to a high professional standard and provide an effective evidence-based clinical service for patients as an autonomous practitioner. This includes advanced assessments for patients with diverse and complex conditions, diagnosing and developing and delivering individualised treatment programmes for neurological patients, as part of a specialist programme of intervention.
  
• To ensure a high standard of care for all patients through effective communication with patients, carers, other team members and wider system partners.

• Demonstrable experience within the Neurological field, with willingness to learn and a passion for improving the lives of people who have recently had a stroke.
  
• Ability to travel across Lincolnshire and rotate between neuro services as discussed at interview.
  
• Autonomous practitioner able to work across acute and community settings and participate in the therapy weekend cover rota.
  
• Full driving licence and access to a vehicle.
