Senior Physiotherapist - Intermediate Care

Job summary: An exciting new opportunity for a experienced Physiotherapist has arisen to work as part of a therapy team at Carlton Court intermediate inpatient care unit based in Lowestoft.

The post holder will be expected to provide specialist clinical skills to manage a caseload of patients using evidence based practice in order to achieve their goals. The post holder will provide a high standard of care as part of a multi-disciplinary team to patients within the intermediate care service and patients own homes. The post holder will be working as an autonomous practitioner applying a high level of clinical reasoning to ensure that individualised care is delivered safely.

Responsibilities
  • Deliver an intermediate care service for patients with highly complex needs within a ward-based setting; may work across wider community settings focusing on rehabilitation and reablement to promote faster recovery, prevent unnecessary admissions and support timely discharge.
  • Assess, plan, implement and evaluate care and treatment with patient-centred, goal-directed care; involve relatives and carers as appropriate.
  • Interpret and analyse clinical facts to form diagnoses and prognoses; develop comprehensive management strategies.
  • Develop and maintain clinical skills aligned with role and registration; provide individualised care plans for a complex caseload and ensure delegated workload is undertaken by appropriately competent staff.
  • Maintain working knowledge of equipment and resources; ensure safe use of therapy equipment and basic wheelchair provision where needed.
  • Participate in multi-disciplinary team meetings; complete clinical documentation and electronic data entry to professional standards.
  • Demonstrate professional standards of appearance, behaviour and attitudes; use clinical judgement in unplanned situations; safeguard patient confidentiality.
  • Lead and mentor staff; provide clinical supervision and support to students and junior colleagues; participate in audit, research and continuous professional development.
  • Adhere to HCPC and CSP standards and conduct; apply clinical governance and risk management principles; contribute to Care Quality Commission (CQC) standards evidence.
  • Promote learning and development within the team and with visiting students; ensure competencies are met for the role.
Qualifications
  • Essential: Diploma / Degree in Physiotherapy; Registration with HCPC; evidence of continuing professional development and management/leadership development; professional portfolio.
  • Desirable: Relevant validated clinical courses.
Skills and Knowledge
  • Good knowledge of physiotherapy in relevant area; forward thinking; excellent interpersonal skills; self-directed learning; ability to work independently and pass on skills to others; good knowledge of CQC fundamental standards.
  • Desirable: Advanced knowledge areas, critical appraisal, audit, research, presentation skills, neurology-specific postgraduate training.
Experience
  • Essential: Postgraduate experience.
  • Desirable: Experience at band 6 level; supervisory skills; experience of a case management approach to care.
