All About You Collaborative Health Care Services LLC, is a Nurse owned and operated, transitional home care agency! Come help us transition patients from hospital/facility to home!! We are seeking PT’s to join our team to help us make a difference in the lives we serve. Our innovative group is a preferred provider for several local hospitals/facilities and Assisted Living Facilities. Our signature LIFE MAP is truly ALL ABOUT YOU.

DUTIES AND RESPONSIBILITIES:

  • The Physical Therapist provides therapeutic services in the patient’s home setting.
  • The services provided by the Physical Therapist will follow a specific written plan of care established in coordination with a physician.
  • The Physical Therapist will work to develop, implement and evaluate the plan of care.
  • Physical Therapist will provide coordination of the plan of care, while working collaboratively with other health professionals. They will identify patient care needs, and make recommendations for additional home health care services if needed.
  • The Physical Therapist instructs, motivates, and assists patients/caregivers in learning and improving activities of the patient's daily living.
  • The Physical Therapist demonstrates timely submission of paperwork and accurate computer documentation.

Requirements

1. Graduate from a physical therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.

2. Currently licensed in the state(s) in which practicing.

3. Two (2) years homecare experience, preferred.

4. Ability to exercise initiative and independent judgment.
