At VNA of Care New England, we’re seeking Licensed Practical Nurses to join our Home Health team, delivering personalized, one-on-one care.


What You’ll Do:

As a Home Health LPN, you’ll provide hands‑on clinical care and support to patients in their homes, while working closely with a multidisciplinary team. You’ll help manage care plans, monitor progress, and ensure each patient receives the compassionate and skilled care they deserve.


Your Day-to-Day Might Include:

  • Providing direct nursing care, including treatments, wound care, and medication administration
  • Monitoring patient conditions and reporting changes to the RN or care team
  • Supporting the development and adjustment of individualized care plans
  • Collaborating with nurses, therapists, and other healthcare professionals
  • Educating patients and caregivers to promote independence and self‑care
  • Accurately documenting all care provided and patient updates
  • Participating in team meetings and interdisciplinary coordination

  • Valid Rhode Island LPN license (required)
