Community Physiotherapist (Rotational post Band 6) at Your Office Toronto. Based at Honor Oak Health Centre, 20 Turnham Road SE4 2LA. You will join multidisciplinary teams to deliver high quality patient care in community settings, with opportunities in D2A, home-based rehabilitation, Urgent Care Response Therapy, inpatient rehab, stroke and neurology (LTC), and the community falls service. Continuing Professional Development is encouraged.

We seek an experienced physiotherapist to join our well-established Community Physiotherapy Team. The caseload will be varied, involving patients from post-surgical onward to those living with complex and long-term conditions, including the frail elderly. You will assess and treat in a variety of settings, and determine whether patients can remain at home with additional support or require hospital admission.

Job Purpose

To provide a physiotherapy service of assessment, diagnosis, treatment and management of patients in the borough of Lewisham.

Job Summary
  1. To undertake all aspects of clinical duties as an autonomous practitioner.
  2. To perform advanced physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions; to provide a provisional diagnosis and develop and deliver an individualised treatment programme.
  3. To take responsibility for own caseload, being responsible for a defined area of the service, with the support of a Band 7/8 physiotherapist.
  4. With the team, to participate in evidence-based audit / research activity to further the team's clinical practice.
  5. To make recommendations to the lead Band 7/8 for any changes to practice by the team.
Specific Duties And Responsibilities
  1. CLINICAL
  2. 1.1 Be professionally and legally accountable for all aspects of your own work, including the management of patients in your care.
  3. 1.2 Undertake a comprehensive, holistic assessment of patients including those with complex presentations / multi-pathologies.
  4. 1.3 Use manual and other assessment techniques and clinical reasoning to provide an accurate diagnosis.
  5. 1.4 Assess patients' understanding of treatment proposals, gain informed consent and work within a legal framework with patients who lack capacity.
  6. 1.5 Use clinical assessment and reasoning with specialist knowledge to provide effective treatment plans in collaboration with the patient.
  7. 1.6 Evaluate progress, re-assess and adjust treatment plans as required.
  8. 1.7 Formulate accurate prognoses and liaise with the MDT regarding discharge planning.
  9. 1.8 Ensure safe and competent use of electrotherapy, manual handling and respiratory equipment; ensure Band 5 staff attain competency prior to use.
  10. 1.9 Use communication skills to interact effectively with patients and carers.
  11. 1.10 Discuss patient concerns and prognoses with patients and families, including those with progressive or terminal illness.
  12. 1.11 Work within Trust, departmental and CSP guidelines; maintain knowledge of national/local standards.
  13. 1.12 In community/domiciliary settings, work as a lone practitioner with telephone support from a senior physiotherapist if required.
  14. 1.16 Participate in an 8am-8pm, 7/7 working pattern.
  1. MANAGEMENT
  2. 2.1 Maintain accurate records for audit and legal purposes.
  3. 2.2 Organise and plan your own caseload to meet service needs and patient priorities.
  4. 2.3 Identify and manage clinical risk within your caseload and area.
  5. 2.4 Comply with Trust policies, Health & Safety, and Data Protection Act; report complaints/incidents promptly.
  6. 2.5 Be familiar with local fire procedures.
  7. 2.6 Be familiar with emergency procedures and infection control; respond as necessary.
  8. 2.7 Ensure equipment defects are reported and addressed.
  9. 2.8 Advise and demonstrate safe manual handling to carers, nursing and staff.
  10. 2.9 Provide training on equipment and condition management when necessary.
  11. 2.10 Make appropriate referrals to other health professionals following assessment/needs.
  12. 2.11 Participate in unit/departmental managerial matters and attend staff meetings.
  1. EDUCATION AND TRAINING
  2. 3.1 Participate in mandatory/statutory training and comply with procedures.
  3. 3.2 Attend in-house training and external courses as appropriate.
  4. 3.3 Contribute to the education of Therapy Assistants when necessary.
  1. COMMUNICATION
  2. 4.1 Participate in staff meetings, MDT meetings and other required meetings, providing clinical opinions.
  3. 4.2 Maintain close links with Therapy Manager and Band 7 Physiotherapist in rotation; collaborate with other disciplines.
  4. 4.3 Maintain liaison with Therapy Manager, senior staff, schools of physiotherapy and other disciplines to promote good working relationships.
  5. 4.4 Provide discharge summaries and patient reports when requested.
  6. 4.5 Participate in IT training and use therapy software.
  7. 4.6 Promote health within hospital and community through example and advice.
  8. 4.7 Handle verbal/telephone complaints appropriately and document them; report to senior as soon as possible.
  1. RESEARCH, DEVELOPMENT AND AUDIT
  2. 5.1 Be involved in clinical/organisational audits to maintain quality standards.
  3. 5.2 Be aware of current research and best evidence relevant to the clinical speciality and incorporate it into treatment.
  4. 5.3 Work with team members to identify needs and develop the service and policies.
