Physiotherapist - Elgin County Area (Adults) at VHA Home HealthCare. The physiotherapist will provide physiotherapy consultation, assessment and planned interventions for diverse client groups across Southern Ontario and the Capital region, referred through VHA Rehab Solutions and Champlain Rehab Solutions. Work within a continuous quality improvement framework reflecting VHA’s vision and mission of services, and promote good public relations with clients, customers and VHA staff, volunteers and associates.

Responsibilities
  • Assess clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA.
  • Implement client interventions based on assessment information, client input and in conjunction with the client’s integrated care plan.
  • Ensure services provided are best practice, evidence-based and evaluated through routine use of valid and reliable outcome measures.
  • Attend client-related meetings to report on assessment findings and discuss plans for intervention, progress reports and discharge as required.
  • Ensure coordinated delivery of care through ongoing communication with all members of the client’s team regarding plans and schedules for interventions.
  • Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the organization.
  • Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health and other agencies.
  • Adhere to the code of ethics of the profession, maintaining standards of practice as defined by the Ontario College of Physiotherapists.
Required Qualifications
  • Baccalaureate or Masters degree in Physiotherapy from a recognized school of physiotherapy.
  • Registered, in good standing, with the College of Physiotherapists of Ontario.
  • Minimum of 2 years clinical experience, preferably in the community.
  • Specialized courses to support areas of clinical expertise.
  • Clinical expertise to provide a variety of physiotherapy interventions.
  • Knowledge of community care requirements and resources available.
  • Excellent consultation and teaching skills, and uses an evidence base in clinical practice.
  • Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.
